The ordering portal is intended exclusively for pharmacies and authorized pharmacy personnel ordering compounded preparations from Crestwood Apothecary. It is not available to patients.

The ordering pharmacy is responsible for ensuring it is authorized to provide any prescription or patient information submitted to Crestwood Apothecary.

Crestwood Apothecary does not sell or rent pharmacy, account, prescription, or patient information.
Information may be shared only where necessary to provide the requested service or where authorized or required by law, including with:
